ALLEGATION(S): 1. Staff handled child in care in a rough manner. INVESTIGATION FINDINGS: On 6/3/26, Licensing Program Analyst(LPA) Anna Morales conducted an Unannounced Subsequent Complaint investigation to deliver the finding of the above allegation. LPA was greeted by Assistant Director Leah Jefferson. LPA toured the facility and observed 23 children under the supervision of six staff. It was alleged that Staff handled child in care in a rough manner. LPA conducted interviews with the staff, children, parents and other parties involved. Based on information obtained, it was reported that a child (C1) sustained a red welt resembling a hand print on the child's left arm. The mark was observed by another party after picking up the child from the day care on May 11, 2026 at approximately at 1:30pm, and at approximately at 3:26pm, the child was observed by a local police officer where the marks were no longer visible. LPA conducted interviews with staff who stated that they did not observe any marks on C1's body before leaving the facility. ALLEGATION(S): 1. Staff are operating out of ratio INVESTIGATION FINDINGS: Licensing Program Analyst(LPA)Anna Morales conducted a Subsequent Complaint investigation to deliver the findings for the above allegation. LPA met with Director Melody Maher. On 2/20/25, and on 2/25/25, LPA observed each of the infant occupied classrooms (2,3 and 7),interviewed 8 staff, and observed that the facility was operating in compliance with teacher to children ratio requirement. Four of the eight staff interviewed stated that facility always remains in ratio. Staff interviewed disclosed that they move from classroom to classroom to ensure that they remain in Ratio and the children are never left unattended. Staff stated that they will call for additional staff if needed whom arrive immediately to ensure they remain in ratio. Four of the eight staff interviewed stated that the facility does not always remain in ratio when the children begin waking up from the afternoon naps(2:30pm). Staff stated that there have been on occasions, that the ratio was 16 children to three staff, and the children wake up and get off from their cots at the same time. 101419.2 Infant Needs and Services Plan a) Prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the the infant. This was evident by; Facility failed to complete a needs and services plan for C1, as required for children who are under 24 months old. This poses a potential risk to the health and safety of children in care.
ALLEGATION(S): 1. Facility operating out of ratio. 2. Facility not cleaned properly. 3. Facility is not meeting day care child's needs INVESTIGATION FINDINGS: Licensing Program Analyst (LPA) Anna Morales conducted an unannounced follow-up complaint investigation and met with Assistant Director Lauren Ferrell. The purpose of today's follow-up complaint investigation is to deliver investigation findings. The investigation of the allegations listed above was conducted by LPA Morales. Based on observations and interviews completed for this complaint investigation, it is concluded that although the allegations noted on this complaint may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. The allegations are thus UNSUBSTANTIATED. A NOTICE OF SITE VISIT WAS ISSUED, POSTED NEAR THE ENTRANCE TO THE FACILITY, AND MUST REMAIN POSTED FOR 30 CONSECUTIVE DAYS. (d) Disaster drills shall be conducted at least every six months. This requirement is not met as evidenced by: Based on interview, the licensee did not comply with the section cited above, LPA was unable to observe documentation of last disaster drill, which poses/posed a potential health, safety or personal rights risk to persons in care.
(1) Except as specified in (3) below, good physical health shall be verified by a health screening, including a test for tuberculosis, performed by or under the supervision of a physician not more than one year prior to or seven days after employment or licensure. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above, S3 and S5 did not have Health Screening and TB in their file, which poses/posed a potential health, safety or personal rights risk to persons in care.
(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above, S5 did not have a current Mandated Reporter Training on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds, 101238(a)(1)The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This was not met by: LPA's observed coackroaches in the infant playground, which poses a potential risk to the health and safety of children in care.
