ALLEGATION(S): Children are being injured due to lack of supervision. INVESTIGATION FINDINGS: On 01/12/2026, Licensing Program Analyst(LPA) Mandeep Kaur conducted an unannounced follow up complaint investigation. LPA met with Director, Dieu-Mi Le and Assistant Director, Breanna Harris and explained the reason for the visit: to gather additional information regarding the above allegation. During today's investigation, LPA interviewed random staff and Director. Additional Pertinent documents were reviewed and obtained during today's investigation. LPA conducted complaint investigation comprising of observations, random staff interviews, random parents interviews, random children interviews, records review and evidence gathered. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the above allegation is UNSUBSTANTIATED. Based on interviews, observations, records review and evidence gathered during the investigation process, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED. No deficiency issued during today's investigation. Appeal rights were provided. Exit interview conducted and report was reviewed with the Director, Dieu-Mi Le.
