ALLEGATION(S): Children are being injured due to lack of supervision. INVESTIGATION FINDINGS: On 01/12/2026, Licensing Program Analyst(LPA) Mandeep Kaur conducted an unannounced follow up complaint investigation. LPA met with Director, D.L. and Assistant Director, B.H. and explained the reason for the visit: to gather additional information regarding the above allegation. During today's investigation, LPA interviewed random staff and Director. Additional Pertinent documents were reviewed and obtained during today's investigation. LPA conducted complaint investigation comprising of observations, random staff interviews, random parents interviews, random children interviews, records review and evidence gathered. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the above allegation is UNSUBSTANTIATED. CCR 101227(a)(1) · Type B

Food Services: (a) In child care centers providing meals to children, the following shall apply: (1) All food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. Each meal shall include, at a minimum, the amount of food components as specified by Title 7, Code of Federal Regulations, Part 226.20, (Revised January 1, 1990) Requirements for Meals, for the age group served. All food shall be selected, stored, prepared and served in a safe and healthful manner. This requirement was not met as evidenced by: Based on observations on 11/20/2025, LPA observed expired Tortillas and bread during complaint investigation in a freezer, which poses a potential Health, Safety, or Personal Rights risk to persons in care.

CCR 101216.3(a) · Type B

Teacher-Child Ratio: (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified,,, This requirement was not met as evidenced by: Based on interviews on 11/20/2025, Director self-admitted that on 11/18/2025, the classroom 9 was out of ratio with 13 children and one teacher(S1) present for a few minutes, right after the nap time, which poses a potential Health, Safety, or Personal Rights risk to persons in care.

HSC 1596.8662(b)(1) · Type B

(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in 1 out of 15 of the staff (Staff #4) which poses potential health, safety or personal rights risk to persons in care.

CCR 101216.1(c)(1) · Type B

(c) To be a fully qualified teacher, a teacher shall have one of the following: (1) Twelve postsecondary semester or equivalent quarter units in early childhood education or child development completed, with passing grades, at an accredited or approved college or university; and at least six months of work experience in a licensed child care center or comparable group child care program. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in 3 out of 15 of the staff (Staff #1, #2 and #5) which poses a potential health, safety or personal rights risk to persons in care.

ALLEGATION(S): Staff hit daycare child. Staff handled daycare child in a rough manner. Staff yelled at daycare child. INVESTIGATION FINDINGS: Licensing Program Analysts (LPAs) Mandeep Kaur and Marilou Monico met with Director D.L., for an unannounced follow up complaint investigation. Purpose of today's investigation: deliver investigation findings. LPAs reviewed the staff files, interviewed staff and parents during the investigation. LPAs toured the inside and outside of the facility during investigation. Based on interviews, observations, and evidence gathered during the investigation process, it is concluded that although the above allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur. The allegations are UNSUBSTANTIATED. Exit interview conducted and report was reviewed with the Director, D.L. Notice of site visit issued and must remain posted for 30 days.
