The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement is not met as evidenced by: Based on observations and interviews, the director did not comply with the section cited above as ants were observed to be present in locations where bibs and diapers were stored and on the ground/walls, which poses a potential health and safety risk to persons in care.

There is a fully qualified teacher directly supervising no more than 12 infants. This requirement is not met as evidenced by: Based on observations and interviews, the director did not comply with the section cited above as the ratio was more than 1 staff to 4 infants and the lead teacher is not a fully qualified infant teacher, which poses a potential health and safety risk to persons in care.

(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed two staff members with a expired mandated reporter training certificate, director stated that Staff forgot to update mandated reporter training.

Every licensed child day care center shall have one or more carbon monoxide detectors in the facility that meet the standards established in Chapter 8 (commencing with Section 13260) of Part 2 of Division 12. The department shall account for the presence of these detectors during inspections. This requirement is not met as evidenced by: Based on LPA's observation, the licensee did not comply with the section cited above (no working carbon monoxide detector on the premises ) which poses as a potential health, safety or personal rights risk to persons in care.
