ALLEGATION(S): Staff did not prevent child from harming other children in care INVESTIGATION FINDINGS: On March 5, 2025, at approximately 8:55am, Licensing Program Analyst (LPA) Maria Olguin-Leon conducted a subsequent unannounced visit to the facility to deliver investigation findings in response to the above allegation that was made to the Child Care Licensing Division (CCLD) on 1/17/2025. LPA met with Director Michelle Lee and explained the purpose of the visit. Present during today’s inspection were 50 preschool age children being supervised by 10 staff. All staff observed during today's inspection have criminal background clearances and associated to facility. During today’s visit, LPA conducted a health and safety inspection, conducted observations, and conducted interviews. During the course of the investigation, LPA conducted both staff and parent interviews, conducted health and safety inspections, and received pertinent documentation. LPA determined that based on the information obtained there is not sufficient evidence to prove the allegation: Staff did not prevent child from harming other children in care. Although the allegation above may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the above allegation are UNSUBSTANTIATED. Upon receipt of this report, Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain postings as required, will result in an immediate $100 civil penalty. ALLEGATION(S): Staff did not prevent a daycare child from biting other daycare children INVESTIGATION FINDINGS: On April 12, 2024 at approximately 3:20 pm, Licensing Program Analyst (LPA) Maria Olguin-Leon conducted an unannounced visit to deliver complaint findings for the above allegation. LPA met with Director, Michelle Lee explained the purpose of the visit. Present during today’s visit was Director and 11 teachers and 52 children. Facility is operating within capacity. During the course of the investigation, LPA conducted interviews, observations and reviewed pertinent documentation provided by facility. Based on evidence obtained, it was determined that the allegation, Staff did not prevent a daycare child from biting other daycare children, is determined to be UNSUBSTANTIATED. Although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. INVESTIGATION FINDINGS: On 07/15/2022 at 8:45am, Licensing Program Analyst (LPA), Kassandra Medrano, made an unannounced visit to the facility to deliver the findings and close out a complaint. LPA was granted entry by the Director, Michelle Lee. LPA explained the purpose of the visit to the licensee. During todays visit, Present at the facility is Director, there were 22 children in care upon arrival. During the course of the investigation, interviews were conducted with staff, parents, and relevant documents were gathered. Based on the interviews and relevant documents, there was not sufficient evidence to prove the above allegation. Although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation did or did not occur. Therefore, the allegation is UNSUBSTANTIATED. Report has been reviewed with Director, and copy of the report will be emailed to the Director. Along with Notice of Site Visit, which shall be posted for 30 days. 101229 (a) (1) Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met, as evidenced by interviews, and documents collected. On 3/10/2022, C1 wandered away from care without staff knowledge. C1 was able to leave facility unattended through unlocked side gate. This poses an immediate health and safety risk to children in care. Although this is a ZERO TOLERANCE VIOLATION which results in A $500 civil penalty. No civil penalty will be assessed due to facility was self-reporting, no injury reported.

A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test.
