Unsubstantiated

ALLEGATION(S): Staff speaks inappropriately to infants in care. INVESTIGATION FINDINGS: On December 8, 2025 at approximately 8:00AM, Licensing Program Analyst (LPA), Katie Krenn arrived at the child care center unannounced to open a complaint investigation into the above allegation. LPA met with the Site Supervisor while the Director, O.L. (D1) was in a meeting. Present during the visit were the director and ten teachers supervising seven infants (12 months and under) and 20 toddlers (12 to 36 months). Later during the visit, LPA met with D1. During the course of the investigation, LPA conducted interviews, made observations, and reviewed pertinent documentation provided by parties involved. Although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. An exit interview was conducted with the Director, O.L. A printed copy of the report and notice of site visit were provided at the conclusion of the inspection. HSC 1596.7995(a)(1) · Type B

(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 9 staff were missing required immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

CCR 101430(a)(3)(A)(4) · Type B

(A) Staff shall place infants up to 12-months of age on their backs for sleeping. (4) Infants with an Individual Infant Sleeping Plan [LIC 9227 (3/20)] that have Section C of the form completed and signed by an authorized representative shall be placed on their back when first laid down to sleep. In the event the infant changes position, the infant may remain in the alternative position. This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above in all 5 infants under the age of 12 months documents were incomplete which poses/posed a potential health, safety or personal rights risk to persons in care.

CCR 101416.2(c)(1) · Type B

(c) To be a fully qualified infant care teacher, a teacher shall have the following: (1) Completion, with passing grades, of 12 postsecondary semester or equivalent quarter units in early childhood or child development education at an accredited or approved college or university. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in 2 teachers were missing required courses which poses/posed a potential health, safety or personal rights risk to persons in care.
