During the inspection the licensor observed dark brown substance (possible BM) on the lid. The licensor inspected the restroom then asked about the spray bottles which were located in the locked cabinet under the sink. Licensor asked assistant and provider whom had come in the restroom what was on the toilet seat and the provider thought the licensor was referencing inside the toilet where the water rings were. The licensor pointed and referenced the seat and asked if it was BM provider replied she was not sure. Licensor requested that they clean it off using the 3 step process. Licensor left out of the restroom.”
