Provider was made aware of two separete allegations of sexual abuse by concerned parent on 2/12/2025 and 2/21/2025. Provider reported to the department but did not follow licensing guidance and WAC to report to intake.

2a) 2 paints labeled “not for children under 3 choking hazard” were in unlocked drawer in Toddler 4 2c) Laminators in unlocked white drawers had loose cord that was capable of forming a loop in Toddler 3 and Toddler 4 that were accessible to children. Phone charging cord was in unlocked white drawers and capable of forming a loop in Preschool 5 and accessible to children. 2d) Plastic garbage bags and diaper wipes labeled keep out of reach of children due to it being a plastic bag were found in Preschool 5, Plastic garbage bags were found in Toddler 3, Diapers in plastic bag were in cubby in Preschool 2, Diaper wipes labeled keep out of reach of children due to it being a plastic bag were on low shelf in Preschool 3, and diaper wipes labeled keep out of reach of children due to it being a plastic bag and garbage bags were on low shelf accessible to children in Preschool bathroom. 4b) Windows in classrooms were 3 ft off ground, within arms reach of children, and opened more than 3 1/2 inches without a barrier or preventative measure discourage children from exiting through the window. 5a) Outlets did not appear to be TR throughout the center behind sliding outlet covers.

Kitchen was unlocked and accessible to children that had bleach container on the floor and cleaning materials under the sink, Destin labeled keep out of reach of children was in unlocked white drawers accessible to children in Toddler 3, and Magic Eraser was in Preschool 3 on handwashing sink
