Numerous teacher reports of a staff member hitting a child on the leg and using inappropriate discipline with children. A teacher reported that she saw the child being hit on the leg. More than one teacher reported that they were in the room and heard the child being hit and the staff member stating she should not have done that.

(2) The Director reported that the they the last time the Health Care plan was udated with the parent, was in 2022. The Director was unaware that the food allergy plan, was incomplete or outdated. The Director reported that staff are trained by looking at the food plan in the classroom trusting that they have. The Director reported that they do not have a system for documenting who had the training by the parent or in house training, (3) The Director reported that they did not give medication or call 911 as the parent was called to pick up the child and was running late when the child was experiencing an allergy reaction to milk.
The emergency exit gate leading out of the licensed outdoor space is not self-closing.
The plastic floor mat in 3-4yr.old classroom is in poor condition, needs to be removed or replaced.

During a Health & Safety Recheck onsite visit, the Serious Violation has not been corrected within 5 business days. Children's Individual Care Plans are still missing information or have not been updated annually. 1 updated Health Care Plan was missing substitution for food allergy. 1 child's file with an food intolerance to soy, was still missing information. 3 children's Individual Health Care Plans, have not been updated annually.
5 files of children with food allergies were viewed. Not all health plans were complete. Some health plans did not have boxes checked for the food allergies. Signatures were missing for the Doctor and the Parent on some health plan forms. The Director or previous Director, did not sign any of the health plan forms. One health plan, form did not have the medication expiration date. 2 health plan forms said gluten free but did not say what foods to substitute or foods to avoid giving to the children. The Director stated the Gluten Free, means wheat. One food allergy plans said milk only, but the Director reported that all dairy products for that child are not given to them as verbally updated by the parent. One food allergy form said eggs, but did not indicate what form of eggs and what not to feed the child. Director said all forms of eggs to include eggs in baked goods. One food allergy form, had boxes unchecked to indicate the symptoms. A child with a food indolence to soy, had incomplete information and does not say why this child cant have soy or what to substitute for soy or what foods to avoid, records are from 2021. Another child with a soy allergy, did not have plans for food substitution or foods to avoid feeding the child.
