During the complaint assessment it was determined, that more likely than not, there was a failure on the part of staff to provide active supervision on the outdoor program space. A child left the outdoor space through a gate that was not the emergency exit and the gate that the child left the space from was not locked. It is reported that neither staff that were outside at the time were aware of what the child was doing prior to leaving the space and were not aware until another member of staff reported that they saw the child running out of the gate on outdoor program space and the child was running towards the street.

Child HA is missing immunizations. 2 other children have incomplete immunization (CIS) forms and exemption (COE) form is not complete.
11) Staf member S.M. was missing first aid and CPR and did not have documentation to show while licensor was on site. 12) Staff Members A.M., M.A. and S.M. were missing food handler cards and did not have documentation to show while licensor was on site.
Gate did not self-close
2c) Unplugged CD player was on plastic storage container in Gayleer classroom with a loose cord capable of forming a loop, unlocked drawer under handwashing sink had charging cord in it capable of forming a loop, air filter not plugged in in Gayleer classroom had cord that was capable of forming a loop. 2d) Diapers in plastic bag packaging were on bathroom shelf accessible to children 3a) Garage doors in outdoor licensed area had splintered wood 3c) Pile of broken bikes were outside in licensed area 4b) Device to prevent window from opening more than 3 inches did not catch and prevent window from opening 33 inches. 4e) Two handwashing sinks (one in Gayleer classroom one in Dayax Classroom) had sinks that reached 122.9 degrees and 124.5 degrees.

Licensors observed multiple packs of diaper wipes and lysol wipes in bathrooms in reach of children and accessible that were labeled keep out of reach of children.
