Bathroom door is observed to be cracked on some areas and must be fixed or door must be replaced. A secondary freezer was observed in the dining room area with no lock and accessible to children. Walls were observed to be dirty and with chipping paint in licensed areas and in kitchen area. Outlet in licensed area are not tamper-resistant. There is blank plate covering the outlets, but they are not screwed on and children are able to easily remove them and outlets become accessible to them. Outlet in bathroom is also not tamper-resistant. Bathroom outlet is GFCI, but is not tamper-resistant.”
