Window blinds in dining room area are observed to be broken and the straps on the blinds are loose creating a loop. Blinds in both licensed areas, dining room and living room, were observed to be broken and accessible to children. Window in dining room is cracked and has tape cover on crack. Bathroom door is observed to be cracked on some areas and must be fixed or door must be replaced. A secondary freezer was observed in the dining room area with no lock and accessible to children. Walls were observed to be dirty and with chipping paint in licensed areas and in kitchen area. Outlet in licensed area are not tamper-resistant. There is blank plate covering the outlets, but they are not screwed on and children are able to easily remove them and outlets become accessible to them. Refrigerator temperature was taken from provider's thermometer and was observed at 42 degrees Fahrenheit. Licensor informed provider that she must turn down temperature until it reaches 41 degrees or under. Cereals were observed to be opened in original container and not in an airtight container with date opened. Provider must place items in an airtight container and labeled with date opened and expiration date.
