Employee R.H. is missing all DCYF required trainings, to include CPR, 1st Aid, Bloodborne Pathogens, Initial 30 hours, Food Handlers, etc. Provder reports employee does not currently serve food. The provider reports she will be obtaining the food handlers card if she begins to serve food.
The second slide was observed to not have 9 inches of woodchips under it. The individual slide appears to have to be lopsided or not secured evenly. CPSC certification for the playground structure and slide or the professional installation information was not available at the time of visit.
