At the time of this inspection, licensor observed registration paperwork for child N.D.T. listed asthma and a nut allergy as known health conditions and the provider did not have a individual care plan on file for this child's health concerns
At the time of this inspection, licensor measured the depth of wood chips surrounding a climbing play structure to be 6" deep.
0165(2)(c) - At the time of this inspection, licensor observed multiple cords capable of forming a loop accessible to children, including an air purifier power cord, a green cable cord located under the white board going toward the teacher's desk in the Preschool room, and a number of computer cables and cords near the teacher desks in both Preschool and PreK classrooms.
0165(3)(f) - At the time of this inspection, licensor observed heavy items such as chairs and plastic storage bins located above the children's cubbies in a loft style platform apx 10 feet from the ground that posed a fall hazard in both Preschool and PreK classrooms. Licensor observed a heavy metal art drying rack located on top of an art supply storage shelf apx 3 feet off the ground that posed a fall hazard in the Preschool room.
At the time of this inspection, licensor observed cleaning supplies stored in an unlocked and accessible cabinet next to the refrigerator and beneath the microwave in the extended day classroom.
At the time of the onsite monitor visit, the playground structure had 0-3" of mulch in all safe fall zone areas.
At the time of the onsite monitor visit, there were two shades with a pull chain that were accessible to children and capable of forming a loop around a child's neck. At the time of the onsite monitor visit, there was one outlet, near the smart whiteboard, that was not tamper resistant.
At the time of the onsite monitor visit, one Aide who was present and listed in MERIT (JFK), did not have a completed childcare basics documented in MERIT.
