0210(1)(2) Licensor observed out-of-date Certificate of Immunization (CIS) forms in student files
Staff member FM was present on site during licensor's inspection. Staff member does not have TB present during the inspection (3)
Cording from equipment (laptops included) in both rooms were observed to be loose and form loops. Plants in the Extended Day room in the window sills throughout were not secure. Outlet in the restroom by sink in the Extended Day roomwas not tamper resistant. It was observed to be GFCI but not tamper resistant. Window sills with stain/varnish no longer present and barewood.
In the lunch room area there is a small tray full of sanitizing spray (Virucide Cleaner) and is accessible to childcare children.
The program was observed to have a child in care that has not yet reached 3 years old and will not be 3 until March. The licensor talked with the director and vice principal about changing the license to the age of 2 1/2 at the starting age instead of 3 or submitting a waiver for the child. It appears through conversation that the program is enrolling 2 year old children. If children are two years old the max group size in a room is 15 with 3 staff.
Staff member Y.G did not have documentation of a Food Worker Card in her file. Director, K. F. did not have documentation of completing Initial Training in MERIT and is required to complete the above trainings listed above under WAC 110-300-0106.
