Based on inspection, video surveillance, and staff interviews it was determined that prohibited behavior and guidance of a child did occur. Staff member's employment was terminated on same day of incident.

Assistant Director's file was not present to verify that she has taken FH training, in Merit, no record date entered. Plastic bags and rubber gloves viewed accessible in 1 pre-k bathroom and in 1 preschool bathroom downstairs. Director corrected the violations during the visit. 3 out of 5 rooms, noted that the windows were open more than 3 1/2 inches, but wide open. 3rd floor PK room, lower floor infant and toddler room. One outlet, had many cords viewed plugged into multi adapter, it does not appear to be a surge protector and cords are not plugged directly into the wall. Surge protector viewed accessible to children by the window in a younger preschool classroom downstairs.

Food allergies and special dietary needs

A written plan for child #1 , has 2 foods crossed out as amended by the parent with milk the only allergy not crossed out. The parent has initialed this. It is not known by the written plan, if milk allergy needs an epi pen. The Director noted that the child is allergic to nuts and one other item that she is not sure of. Director reported being unaware of the amendments. No written plan for foods to substituted for this child viewed completed. A written plan for child #2, has a written plan for a food allergy with Benadryl first and epi pen second to teat an allergic reaction. No Benadryl was provided to the provider and only an epi pen was observed in the backpack . No written plan for food substitution observed as the section on the form was viewed blank with no foods entered. 2 children with food allergies, the provider must arranged with the parents to have the necessary medication, information, and training to properly manage a child's food allergies.

Infant safe sleep practices

In one Childs file is a waiver was observed from a parent stating she is waiving WAC 110-300-0291 and allowing her child to sleep with toys or stuffed animals. The provider must follow safe sleep practices according to WAC. Waiver was observed signed by the parent and on SIDC letterhead. No official wavier from the Department has been approved to allow toys or stuffed animals at sleep time.

Electronic cords are accessible to children in all classrooms that could form a loop around a child's neck. Metal chains in open hallway cabinet, were viewed accessible, several formed a loop as they were attached to the cabinet, several were viewed to not form a loop as they were loose. All chains posing a choking or entanglement hazard. Push pins on bulletin board in hallway viewed accessible to children and poses a choking hazard. Duct tape accessible to children viewed in older toddler room posing a choking hazard. In Preschool room, loose tape on wall accessible to children, poses a choking hazard. 2 extension cords viewed with appliances plugged in. One in the infant room with a fan plugged in, was brown extension cord, reported by teachers not for temporary use. One in the early toddler room had multiplie appliances attached, viewed white and reported to not be temporary by staff. Both are extension cords and not power strips. Broken silver metal type bracket sticking out of the wall and observed accessible to children, poses a cut or puncture hazard. Adult scissors observed in the toddler room, accessible to children above their cubbies, posing a cutting hazard. Plexiglass on diaper changing stations, viewed cracked. Cracks start where the screws are and run throughout the plexiglass. Objects in the hallway open cabinets, cloth bins with materials inside, pose a falling objects hazard as they are no securely stored in the cabinet. Strollers, car seats and random stored supplies, are not secured and pose a falling objects hazard. Objects in the older toddler room, viewed unsecured an on top of the children's cubbies, all pose a falling objects hazard. Objects in the Pre-School room, viewed staked up high in piles, all accessible to children and poses a falling objects hazard. Electrical cords viewed plugged into extension cords. Surge protectors viewed in classrooms and are accessible to children. Electrical devices viewed accessible to children, are viewed plugged into the outlet next to the handwashing sinks.

Emergency preparation and exiting

Exit doors in hallway connecting 4 classrooms, viewed partially blocked with large wide tables with furniture stored underneath, open cabinets with materials stored spilling outside of the cabinet, next to a door in egress to fire exit.

Director says she does not have access to the fire safety annual inspection. In the pre-k room, a large plastic container full of stuffed animals, was viewed next to and touching a wall heater.

Director stated that her file was missing documents and that she could not locate where the previous Director put them and it is the same with other staff filre. 2 videos were viewed of teacher and child interactions. Video 1: showed a teacher restrict a child's movement by pinning a child to a mat at rest time with her leg, showed teachers slapping children on the back at rest time using full arm motion, a teacher pushing a head of a child down multiple times with her hand when a child would move their head up to look. Video 2: showed a child follow a teacher to a gated area with the teacher physically slamming a gate in the child's face to stop the child from access into the gated area. Although the child appeared not hurt but stunned, the teacher interaction itself, was abrupt and hostile as the gate was slammed in the child's face and the child was ignored by the teacher.
