One staff member, who has been registered with the facility for over two years as an assistant teacher, does not have Child Care Basics (CCB) listed as completed in Merit. The director reported that the teacher did finish the class, but due to language barriers and other complications, the completion was not recorded in her account. Additionally, there was an error with the email provided during registration, so the certificate was never received, and no STAR ID was added.
Plastic bags were observed within children's reach in the "Little masters" room, both on the floor under the sink and stored in boxes on top of a low shelf, making them accessible to the children.
The medication authorization form for the child with asthma was observed to be expired.
0146(1) - The Caterpillar play structure was positioned directly beneath the tall climbing equipment, obstructing the designated safety fall zone. 110-300-0146(1)(a)(b) - The wood chips ground cover in the fall zone beneath the slide of the tall climbing play structure was compacted and did not meet the required 9-inch depth. Another slide was observed in the "Little Genius" classroom, positioned in the middle of the play area and surrounded by soft blocks and mats.
