At the time of inspection, this licensor observed a storage closet located in the "main room" to be unlocked with multiple accessible first-aid kits. These first-aid kits contained alcohol sanitation wipes, burn creams, antiseptic wipes, non-aspirin Acetaminophen packages, and loose white pills. This door did not have a way to be locked.

Provider's Food Worker card expired in 2/9/2023. The licensor pointed out to her that the information even though verified in MERIT was inaccurate that a Food Worker card are usually is good for 3 years max but not usually for 4+ years. The food handler card that was verified in MERIT read 2/9/21-3/12/2025. The licensor discussed with Provider that her Food Handler card would have expired in February 2024 if it was a 3 years one. The one in her file read 2/9/21-2/9/23 and that although verified it is not accurate information. BBP for the provider expired 2/18/2023. Provider had a certificate for 20 hours STARS completion from 12/4/2010-12/5/2010 but it is not reflected in MERIT told to follow up with MERIT because currently the MERIT system reads 0 for Initial Basic Training.
