The 2 seated structure with steering wheels has rusted screws and chipping paint. The "store" sign near the back fence has green chipping paint. The posts in the back fence have green chipping paint. The gate along the back fence has nails sticking out.
In room 119, there was a epi pen box that did not have the authorization form. The box for the epi pens had an expiration date of 9.2024 and the acutal epi pens had an expiration date of 3.2025.
Rooms 107 and 114 had cables cords hanging from the screen and they are capable of forming loops. There were plasic bags accessible to the children in rooms 107, 114 and 119. The yellow handles on the wooden play structure are rusty.
Shital Patil was preparing snack and she does not have a food card. Carmen does not have CCB's. Carmen does not have children experiencing homelessness training. Kellina does not have medication management and administration training.
