During the observation, Licensor Harris did not observe an individual care plan for child WVH that addressed substitute for allergenic foods, specific treatment plan in response to an allergic reaction (1)(b)(c)
During the inspection, Licensor Harris observed brown sugar in an airtight containter that was not labeled or dated, three bags of shredded cheese that were not labeled or dated in a ziploc bag, 6 yogurt containers that were in an airtight container not labeled or dated, hummus that is in original container but not dated when it was opened. There were zip loc bags of veggie patties in the freezer with a date, but they were not labeled with contain name. (4)(a)
During the inspection, Licensor Harris observed a tied plastic bag with garbage in the infant/toddler playground (2)(d) During the inspection, Licensor Harris observed the following toys that were broken. In the infant and toddler playground, there was a broken kitchen play set. In the preschool playground, there was a broken kitchen play set (3)(c) During the inspection, Licensor Harris observed a broken wall panel or board in the toddler bathroom near the toilet. Panel or board was placed near the toilet against the wall. (3)(g) During the inspection, Licensor Harris tested water at the sink inside of the school-age classroom. It tested at 66 degrees Fahrenheit. The sink in the preschool classroom tested at 60 degrees Farhenheit. (4)(e)
