There was a charger on the floor in the nap room that has a cord capapble of forming a loop and is accessible to the children - provider removed during visit. In the backyard the gutter spout is detached/broken from the gutter and it is accessible to the children. The new play structure has openings in the "wobbly steps" and the plastic "hole openings" that are entrampments. The holes measured 4in. wide and 6in in length. This does not meet page 9 of the playground handbook: Prevent Head and Neck Entrapment Play equipment should have no openings between components that could trap a child’s head or neck. · Check all openings on the play equipment – such as between ladder rungs, posts, and railings. Be sure spaces are either smaller than 3½ inches wide (so children can’t put their body through) or larger than 9 inches (so children can slide their body and head through). There was an adapter/electrical cord with something plugged in it. They both had loose cables and it was not plugged in, they were accessible to the children - provider removed during visit.

Storage of hazardous and maintenance supplies

There was a large shovel in the backyard that was accessible to the children. There were 2 large wheel barrels in the backyard and they were accessible to the children.
