Wood chips were observed to be less one inch throughout the area. Tires and other item within the 6 foot fall zone. Licensot to staff fall zone with Licensoing supervisor and Health Specialist

Fire extinguisher observed to not have been updated or serviced within 12 months. Annual fire inspection was not available at the time of visit. Fire extinguishers were serviced and corrrected the same day.

Employees BK and TF First Aid and CPR expired 1/29/24. No food handlers card was observable for employee SF. Technical assistance was provided to complete. Bloodborne Pathogens yearly and input all education documents into MERIT. Safe Sleep appeared to be expired for BK and TF. Provider reports they had completed it but i was not showing on MERIT. Technical assistance was provided, provider was able to obtain certifiacte.
