An infant was sleeping in a room with a door closed which prevented them from being in sight range of provider. This was corrected during licensor's inspection.

There was an emergency exit door that was locked from the inside with a deadbolt. There were two baby gates in emergency exit pathways whose locking mechanism required special knowledge to unlock and did not open with a single motion.
