There was a phone on the wall by a handwashing sink with a cord hanging in Aspen/Room 1. The cord was accessible to the children and was capable of forming a loop. There was an extension cord, cord to an air purifier, phone cord, and a laptop charger (all capable of forming a loop) accessible to the children in the library room. The incident report was not provided to the parent on every occasion when the child sustained an injury. The incident report itself did not meet the required standards, missing a signature section and failing to cover all necessary details of the injury. WAC 110-300-0475(2): The program failed to report to the department when the child sustained an injury to his head, and the parent took the child to urgent care and later to the emergency room.
