At the time of the onsite inspection, there were two three-step slides (one blue/green, one blue/red) that did not have an installed safe fall zone material.

At the time of the onsite inspection, the front door had a shade installed that used a chain pull which formed a loop and was accessible to children. At the time of the onsite inspection, the TV was on a stand approxmately 18" off the ground, accessible to children, and able to be tipped easily, posing a tipping hazzard. At the time of the onsite inspection, the two outlets on either side of the kitchen sink were not GFCI outlets and were not able to be verified as protected by a GFCI outlet.
