Licensor observed the following: A cord from the "open" sign to be hanging near the window where children eat and is observed accessible to children. The cord is long enough to wrap around a child's neck. A water hose in outdoor space that was left spread out causing a tripping hazard. Provider corrected this while licensor was on-site. A black speaker that was placed on top of a cubby area. The speaker was not secured and is observed to be a risk of falling on a child. Outlets in bathroom were observed GFCI, but not tamper-resistant.

Licensor observed two outlet covers broken with sharp edges. Only one was accessible to children. Licensor observed outlet in one room that is not tamper-resistant and outlet is accessible to children. Licensor also observed bathroom outlet near sink to be GFCI, but not tamper-resistant.
