It was observed that there were 6 wooden stakes in the ground that pose a hazard. It was observed outside there was a climbing structure with an age recommendation for ages 3-6 years. Provider reported that children under the age of 3 have used the structure. It was observed that there was a 4 inch gap under the outdoor gate.
