The license did not produce staff files as they were stored in an unlicensed portion of her home. S.D. was not aware of 110-300 required ratios as well as the necessity of keeping gates to the kitchen latched.

This licensor requested that the provider keep the gates to the kitchen closed multiple times. S.D. was observed to leave the room on two occasions with the gates open. S.T. was not in the room when S.D. did this.

A provider who no longer works for the program was still listed as an Assistant Teacher in MERIT. S.D. began working as an Assistant after completing her high school education. However, she was still listed as a household member in MERIT. This information was updated prior to my departure. The provider and her assistant's staff records were completely unavailable for review. She reports they are stored upstairs in an unlicensed office. The only documentation available for review today was the licensee's current First Aid, CPR and BBP training which she had on the bulletin board in her licensed space.

Kitchen knives were accessible in a kitchen drawer and on the kitchen counter and older preschool children, who could reach the drawer and counter-top were in care today. The kitchen gates were not latched to prevent child access. After informing the provider the gate needed to be latched at all times, I observed it unlatched two more times.

The provider reports she requested a fire inspection this year. They declined. She did not complete the WSP FH Fire Inspection form on her own. The provider does not have smoke alarms in the smaller room with two cribs, and the larger child care room near the front of the house. Both of these rooms are used for sleeping.

Upon my arrival, I was greeted by Assistant S.D. F.D. was in the garage and outside warming up his car to depart for the day. Seven children were in care, some younger than two years. S.T. was upstairs in the unlicensed portion of the home. I noted that she needed to be working with a qualified individual to be in ratio. She was suprised by this stating she thought that she could serve up to eight children by herself. I provided t.a. on this. Within a few minutes, F.D. came back in from the garage to asssist.

Child I.C. left the main child care room to enter the back room where this licensor was working. He opened the gate to the ktichen and wandered within it for a few minutes. I got up to move toward him and he exited the kitchen shutting the gate behind him. After five minutes total, it was noted by S.D. that he was not with the providers and group of children in the main child care room and she called him to return to the group. He did so.

A child's epi pen was stored without a medication authorization form. The medication form stored in their file was over one year old. The epi pen expired at the end of 2022. When I asked the provider to return it to the parent at pick-up today, she attempted to put it in the child's cubby which is located at child level.

The provider has acquired a new air hand dryer. The cord is accessible to pre-school age children. Stairwell is not blocked and toddlers were in care today.
Emergency preparation and exiting

The provider's primary fire exit through the sliding glass doors at the rear of the home was observed blocked today by an infant swing and children's table and chairs. The East side of the main child care room does not have a four foot exit path. The provider has blocked this room by a shelf that allows approximately two feet of egress on each side, and a baby gate that leads to the kitchen,and the rear fire exit.
