Exit door leading to outdoor play space was partially blocked with a child proof door guard device. This prevents an emergency exit from building. This was discussed on site with R.B.

During annual inpsection, Licensor A. Stewart observed playground equipment, rock climber, slide and tires did not have nine inches deep ground cover. Director C.D. reported they are aware, they are in the process of ordering turf.
