(1)(2) 2 child files that were reviewed had CIS forms from 2023, 1 child file that was reviewed had a CIS form that was not medically verified, 1 child file that was reviewed had a CIS form from 2023 that physically filled in and didn't have a list of immunizations, and 1 child file that was reviewed had a blank CIS form.
(1)(a) There were wipes labeled "keep out of reach of children" accessible to children in the Toddler A classroom, Preschool classroom, Pre-K 1 classroom, and in the playground by the front office. There was a disinfectant bottle and a unlabeled bottle with blue liquid accessible to the children in the Preschool classroom.

(3)(d) The storage/laundry room was unlocked and accessible to the children. (3)(g) The toilet in the bathroom to the right in the Preschool room is rusting at the bottom and needs to be fixed.
(2)(a) A wall in the Preschool room is peeling and has paint coming off. The door in the back of the room is also peeling. Both need to be fixed so that they can be cleaned properly. (2)(f) The center is using something other than bleach to clean and it has not been approved from the department. There is no waiver in place in the provider's account.
There were two epi-pens in Pre-K 1 that are expired. There are some diaper creams and lotions that are stored together. There are diaper creams accessible to children in Tod 1.
0165 (2 d): There was a roll of plastic bags on the counter in Pre-K 2.
There is a gate between Tod A and B that has a door knob that requires special knowledge and is only accessible on one side of the room.
