Observed chipping paint on walls in toddler room. Observed the "ship" sandbox mesh cover torn and broken plastic cover on the bottom of sandbox accessible to children. One gutter downspout opens to the preschool playground at child height. It needs to be redirected so dirty water and sharp edges are not accessible to children.

Chipping paint/exposed wood was observed indoors during visit. The outlet in the girls bathroom was not observed to be GFCI. The climbing dome was observed to have chipping paint and rust in areas accessible to children. The kitchen gate was observed open, making the area accessible to children. The toddler playground has a large storage box that was open, the fridge and freezer child safet latches were not in use, resulting in potential entrapment hazards. Shed door was not locked. Sand box was observed to have a broken cover with sharp wood. Garbage was observed throughout the grassy areas of the playground and in the sandbox.

Woodchips were observed to be less then 9 inches. The castle slide and climber, and the blue slide with stairs were observed to be on the grass and not on proper ground cover. ground cover in toddler playground is not 9 inches.
