The licensor observed a pull wagon used for storing craft items positioned near the exit door. The licensor provided guidance that exit pathways must always remain clear and unobstructed to ensure safe and immediate evacuation in the event of an emergency, and advised relocating the wagon to a designated storage area away from exit doors.(immediate corrections made: The pull wagon was removed from the exit egress.).

Files were not onsite for review in order to verify training. Files were not accessible to review at the main office where files are located because the Director had appointments.

Cording by winoow in circle area was observed to be loose and accessible. The cording from the extension cords were loose and could form loops. Heavy plants on shelves and items (tool box) stored on top of shelving needs to be secured. Outlets in restrooms were observed not to be GFCI nor tamper resistant. Extension cords observed being used extended time and plugged into outlet.
