Prohibited behavior and discipline, and physical separation of children

When interviewed the provider acknowledged that he had "grabbed" and held the child's hand twice to prevent him from saying negative things to other children-once immediately before departing the Center, and twice, during the van ride to school. This licensor observed that the provider had long fingernails. Although he describes his hold on the child's hand as "gentle" and "like a handshake", a scratch and red mark were observed on the child's hand by a referrer shortly after arriving from school. The child also alleged that the provider had grabbed him. This licensor concludes that, more likely than not the child's hand was grabbed and the mark and scratch were the result.

Laundry room in hallway was unlocked and unsupervised by any provider. Fan was placed on shelf in school-age room. Cord was exposed and fan could easily be pulled down, or fall down and injure a child.

Two providers, Kayleigh and Jannette, have been employed longer than three months and have not completed CCB or an equivalent. Two providers, Sawyer and Hana, were working in ratio today and not have First Aid, CPR and BBP training. The Director acknowledges that she was not aware that all staff need BBP annually per L & I requirements and many providers do not have annual BBP entered in MERIT. Multiple staff have expired Safe Sleep training, Kyleigh, Amber, Nylani, Yeimi, Nikia and Natisa. None of these women worked in the infant room today.
