Licensor found that a child had been brought into a classroom as the rest of the classroom was going outside. The staff member in the classroom did not observe this child get out of line and go into the restroom. The staff member then did not notice the child did not come outside with the rest of the group.

The incident report provided for the child from 1/5/23 does not reflect multiple bites, and states the child was bit one time. The teacher in the classroom at the time of the incident was unable to be spoken to due to a medical leave. The admin who signed the incident report explains she was aware they child had been bit multiple times but did not realize the extent of the bites or the staff member present at the time of the incident. The center reports they try to support the classroom during transition times and diaper changes in effort to support with supervision.

During an onsite complaint inspection completed on 11/14/23, Licensor Anselment and Whitaker, along with Center Director Miller were watching live feed video footage of the preschool classroom. During the observation three children were observed not washing their hands after using bathroom during a ten minute time period. Staff were not observed providing handwashing guidance and no handwashing was completed prior to the children returning to classroom activities. Center was Staff retraining on handwashing was previously cited for same WAC on 9/25/23 during a Monitor Visit. reported to have completed on 10/13/23.

Video shows the teacher forcefully carrying the child, grabbing the child's arms and holding the child down. Staff report the teacher was using threats and harsh tone.

On 11/6/2023 Licensor Anselment and Whitaker observed center employees Kyla Alcorn and Lydia Corrigan in the Pre-K classroom together working with children. Lydia Corrigan is currently on a safety plan and is to be paired with a qualified staff member. Kyla Alcorn did not have Bloodborne Pathogens training at the time she was observed with children, per WAC 110-300-0106(10) employees must have Bloodborne Pathogens training prior to working with children. Center director reports she was aware she did not have the required training and she would be completing it.

Video footage shows multiple incidences where teachers are not actively supervising children. Staff report during the previous months staff frequently was not actively supervising children. Staff would often talk to each other through the sliding doors, be on their telephones or not paying attention to children.

This licensor reviewed the parent handbook and diapering policy. The policy states diaper changing will happen every two hours. During staff interviews, one teacher reported incidences of not changing diapers in the toddler room due to being on medical light duty and having an injury to her hand. She reports she would be in the classroom alone and would need a second staff member to complete diaper changing. When she would notifiy the office to have someone come complete diaper changing most often no one would come for 30-60 minutes.The staff member reports frequently diapers would not be changed every two hours due to this.

The detective provided licensor still footage from the video of the employee pulling the child by the shirt and restraining another child in her lap by holding the child down with her legs. During licensor the onsite the employee was in the toddler 1 room sitting with toddler attempting to put the child down for nap. The employee reported she could not get children to sleep because they could not lay down willingly and requested for additional support.

On 9/25/23 Center admin staff reported all children in the young preschool 2 classroom were over 2 ½ years of age and therefore the classrooms were running at a 1 to 10 ratio. Upon touring the room Licensor Anselment viewed one teacher with 8 children. Licensor requested child files for the children in the classroom to review birthdays. Upon review of child files of it was determined two of the children were under 2 ½ years of age, both having birthdays in May of 2021. This classroom was determined to be out of ratio for the morning and operating at a 1 to 10 ratio instead of the required 1 to 7 ratio. The Licensors had been onsite for over an hour before the ages were verified and ratio was corrected. The center director reports there has been occasions when the center was out of ratio due to call outs and being short staffed. Upon viewing video footage there was once incident viewed where a teacher was out of ratio for over 20 minutes.
