Licensor observed 16 children and one teacher in the classroom which capacity is 14 children.

0186(1) - The individual care plans observed did not meet the following requirements: (a) Some plans did not identify foods that cannot be consumed. (b) Some plans did not specify foods that can be substituted. The forms were not signed by a healthcare provider. Some forms lacked complete information regarding reactions, treatments, and end dates. Not all medications provided for children with allergies were accompanied by the necessary forms. 110-300-0186(2) - A child, in #102 classroom, that have an allergy did not have medication provided by their parents.
0165(2)(c) - Cords were observed at child-accessible levels in multiple classrooms. 110-300-0165(5)(e) - A power strip was observed on a table accessible to children in classroom #101.
Eight teachers did not complete the required 10 hours of continuing education in 2023. Four other teachers did not complete the required 10 hours of continuing education in 2023, but can complete a few additional hours in 2024 to fulfill this requirement for 2023.

There was a child in the Yellow Duck room that had an allergy with no individual care plan. The medication form was not filled out with the stop date on it.
There was a child in the Yellow Duck room that has an inhaler that did not have the prescription with it. The medication form was expired. In the Brown Bear room the inhaler for a child had expired 10/2023.
Cleaning supplies were stored by the kitchen sink and the kitchen was open and accessible to children.
Exit doors in 3 of the classrooms were locked with child safety locks from the inside of the classroom.
