Not all staff persons had completed child care basics and the federally mandated trainings within 3 months of hire. One staff person had expired CPR/First aid training.
In a preschool room, the cubbies were in poor condition and had chipped paint. In this same room, and outlet had a burn/scorch mark on it.
One child with a food allergy did not have a care plan signed by their doctor. The care plan also did not list appropriate foods to substitue.
Two diaper creams did not have authorization in the waddler room. These diaper creams were prescription and one was not stored with the prescription label. One EpiPen also was not stored with the prescription label.
In the infant room, the program did not have a "Water" bottle to 3 step clean. In multiple classrooms, toilets had visible grime and what appeared to be mold inside the bowl.
One child's allergy plan did not have substitute food and a doctor's signature.
