Cording for equipment in rooms was dangling in the toddler and preschool room and/or accessible to children. Need to ensure that heavy items on shelving units are secure in rooms. In the toddler room a screw used to possibly secure shelving together was protruding out. Outlet in restroom was observed to not be GFCI and was by the sink.

There were no drills documented for September 2023, November 2023, December 2023, January and February 2024. The drill log sheet was from 2023-2024. There was no lockdown drills noted for this year thus far. The drills noted on the sheet were 1/2023, 6/2023 and 11/2023 and 1/2024 Drills are to be conducted monthly for fire and quarterly for Disasters.

Staff member, I.B.A does not have the medication managment training. Medication forms needed for ointments in Toddler Room and 1 child had a medication form that had an ende date but the ointment was still being used in the waddler room. In the toddler room the medications were stored all in one bin external and internal. There was also expired medication in the toddler room that expired in 2021.
