Supervisor, JB, observed a metal turtle in bathroom used for children and staff that had small circles smaller than one and three-quater inches available children. All bookshelves and cubbies in licensed upstairs space were not secured and are unstable creating a tipping hazard. Wires in licensed area upstairs and downstairs were observed loose and accessible to children. Water temperature in bathroom was observed at 127 degrees Fahrenheit. In outdoor space, licensor and supervisor observed garden scissors accessible to children. Extension cords were observed in upstairs area that were plugged in for lights around the edge of the roof. Washer and dryer were accessible to children with no lock preventing the children from entrapment.
