Two toddler slides were placed on an inch of play chips, without adequate fall/use zones around them. These were removed immediately. The wooden climbing structure with slide (treasure island) does not have an adeqate fall/use zone or an adequate depth of play chips under and around the structure. a playhouse was within its fall/use zone- 5 feet.

The wooden climbing structure does not have adequate fall zone or fall material around it. A lounge structure was placed within the fall zone. There is also a plastic slide climber that does not have an adequate fall zone.
