MERIT was reviewed prior to going to the site for the visit and there are numerous staff missing current Safe Sleep Training. Files reviewed an in MERIT almost all staff are missing documentation of current BBP training. Review of MERIT only reflects 1 staff member that has a current BBP training. Staff completed CPR/First Aid according to the Director, M.P.F and Program Supervisor, L.C. but none of them have received their cards as documentation of the training. Follow up will happen again with the trainer to obtain copy of the certifications. Staff were missing 20 or 30 hour STARS reflected in MERIT. Staff without the 20/30 hour STARS certificate will need to complete the 0106 Health and Safety Trainings and/or 30 hour STARS. STaff files were missing documentation of Food Handler's Permits (L.C; M.S; and D.P).
On the ramp on the preschool play area structure there is water pooling on the ramp area. Add more woodchips under drop areas under slides and swings.
Cording from equipment was observed loose and dangling in some rooms. Plastic bags were observed in the school-age room in drawer accessible to children. Handheld can open was in a drawer accessible to children. In rooms throughout ensure that heavy items on shelving units are secure (CD Players, Air Purifiers, Computer Equipment, and Milk Crates. Rooms shelving in some rooms and chalk board tray in school age room was observed to have chipping paint.
A child has an epipen that is expired as of 12/23 and the child also has a medication listed on their individual health care plan that was not provided to the site (Cetirizine).
Diaper ointments in classrooms were observed to not be labeled with the first and last name.
Items stored under sink (soap refill) was accessible to children.
