The provider has requested that the parents complete the form acknowledging that they have read and reviewed the parent policies and handbook, but reports these sign in sheets have not been returned to her. She does have verification from parents via some of the policies.

Government i.d. was missing in one staff person's file.

The Staff Policy Handbook does not include the following: -Job Descriptions particular to the child care -Early Learning Program staff responsibilities for: supervision; including preventing children's access to unlicensed space; child growth and development; developmentally appropriate curriculum; teacher-child interaction; safe sleep practices; child protection, guidance and discipline techniques; food service practices; health, safety and sanitation procedures; medication management procedures; implementation of child's Individual Care Plan; religious equity and cultural responsiveness; planned daily activities -All of the information in the parent or guardian handbook -Staff responsibilities if the Center Director, Assistant Director or Program Supervisor is absent from the program. -A plan that includes how adminstrative and child caretaking duties are met when a job requires such dual responsibilities. The current staff poicy does not contain information relevant to Chapter 43.215, Chapters 110-300 and 110-06 WAC.Therefore, staff have not been trained on all of these WACs and RCW and the training was not developed.

One of the fire exit in the toddler room has a door that has warped and was extremely difficult to open. The door was able to be adjusted to be closed but still easy to open while I was present.

Two bottles of bleach/water sanitizing spray was observed accessible to toddlers on the toddler diaper changing table. These were moved immediately upon my request.
