Outlet covers smaller than 1 and 3/4 inch were in place on outlets accessible to children in an infant classroom. The outlets are already TR type. The covers were removed immediately. Plastic bags and gloves were accessible to children in under sink cabinets in several classrooms. They were made inaccessible immediately.

Three providers have been working in child care for more than three months have not completed Child Care Basics or the federally required months. Isopropyl alcohol was observed accessible t children in the cabinet under the sink in Primary 3, Room y. All were made inaccessible immediately.

The laundry room/janitorial room was observed open, the door propped by this licensor. It contained bleach, cleaners, a wet mop, a vacuum and open washing machine and dryer doors. It was shut and locked immediately.

When interviewed, a provider reported that she had exceeded the required ratios for preschoolers for "most of the week". The Director acknowledged that not all providers were following the Center's procedures for avoiding exceeding ratio.
