0165(2)(d) One of the bathrooms had a large plastic bag covering the toilet seat. There were some plastic bags accesible to children under the diaper changing table. All bags were removed during the visit.
Bottles in one infant room was missing dates.
Need to ensure that medication forms for diaper cream and sunscreen is completely filled out (expiration dates, start and stop date etc.) signed and dated with day and year. Some staff are missing Basic/Initial STARS and upon review do not have the DCYF medication training. In Room 111 it was observed that there was moisturizing cream for several children by the sink and accessible to all children.
Roller shades on windows had hardware on the wall to manuever cording but the cording was still loose. Equipment cording in rooms observed to be accessible to children and can form looping. In rooms heavy items stored on shelving units and above cubbies were not secure. Chipping paint was observed in door frames, window sills, and on montessori number rods and blocks. Room 109 water exceeded 120 degrees degrees one sink was at 125.2 and 122.9.
Room 111 was observed to have a small bottle of Dawn soap and a Magic Eraser sponge by the sink accessible to children.

F.A was missing Basic/Initial STARS. She completed Safe Sleep listed under WAC 110-300-0106 but is missing all the rest of the Health and Safety Training via the DCYF Portal listed under WAC 110-300-0106, All staff files were missing documentation of a current BBP training.
