Equipment and surfaces in outdoor early learning space

There was equipment that requires proper spacing. Program has swings and the required spacing is not between the infant/toddler and older children swings. Swings will need to be used 1 at a time. There was a play structure/climber and slide (Kangaroo Climber) that was on grass and not in the wood chip area. When and if the equipment is moved a 6 feet minimum spacing is required between the equipment. The Kangaroo Climber also had an area when facing the equipment the right wall was not in the joints. Wood chips needed to be added under swings and other drop areas on the playgrounds.

In the unlicensed/unapproved Infant Room (Trailer) bottles were observed to be missing last name of the child. Diaper area is not 8 feet away from the food prep area and there is no barrier between the areas to prevent cross contamination.

Some diaper ointment consents are missing information from the medication authorization form (amount, when to apply expiration date, child's DOB, end date). A child had a Tylenol medication authorization form that was missing information which includes why the Tylenol is being give to the child. Medication/Diaper Ointments were missing last name. In the Preschool Room there was sunscreen under the bathroom sink and the cabinet was not latched which allows children access.

During the unannounced monitor visit on 8/23/2023 the licensor was notified that the program had moved Infants due to planned construction out of the licensed building to a unapproved licensed Trailer on the premises. The trailer has not been approved by the Department nor the State Fire Marshal. The space did not have a Occupancy Permit for the Trailer when visits were conducted by the previous licensor, Diane and supervisor, Natasha and the fire marshal, Cozetta.
