Multiple classrooms had phone cords hanging off that were accessible to children.
The director was unable to show Licensor the most recent water testing results.
There was no record of staff medication training for children with epi-pens.
There were diaper creams in several of the rooms without full names on the containers and some without names on them at all.
There were outlet plug ins in the infant room and toddler room that could be a choke hazard. There is a tall teacher's cabinet in room 8 that is not secured to the wall and there are objects on top of it that could fall.
There is a gate that is blocking the exit in the infant room. In classroom 2 the second exit is blocked by stuff that is in room one by the door.
The bleach bottle was accessible to children in room #9.
There was a blanket in the crib of a sleeping infant. This was addressed the day of visit 6/9/27with a safe sleep violation letter signed by director and was posted for parents, verbal notification to director and in person notification of violation to office manager. Director also will go over safe sleep WAC again with all infant room teachers.
