In the Otters classroom, the medication observed for one child, provided by the parent for an allergic reaction, was not the prescribed medication listed in the doctor’s medication form and was not appropriate for a 5-year-old in the 5ml dose indicated. In the Raccoon classroom, the medication authorization form listed a doctor-prescribed medication for allergic reactions that was not provided to the program by the parents.
