0166(3)(c) Emergency exits had baby gates installed in them. As immediate fix, the provider agreed to keep the gates open until they can be permanently removed as they are affixed to the door frames.

0186(1): Medication authorization forms for prescription medications were either not updated or were missing essential information, such as doctor's signatures, names, end dates, etc. -
0186(2): In the Otters classroom, the medication observed for one child, provided by the parent for an allergic reaction, was not the prescribed medication listed in the doctor’s medication form and was not appropriate for a 5-year-old in the 5ml dose indicated. In the Raccoon classroom, the medication authorization form listed a doctor-prescribed medication for allergic reactions that was not provided to the program by the parents.
0165(2)(a): Tamper-resistant covers were observed in the TR plugs in Racoon classroom. The director removed the plugs and provided TA to the teachers, asking them not to use these covers. -
0165(2)(c): Cords were accessible to children in several classrooms. The director covered some of the cords with shelving during the monitoring visit. -
0165(5)(e): A power strip with several cords plugged in was observed in the Raccoon classroom, located in the corner and accessible to children. -
0165(3)(c): A cracked toy with a sharp edge and pinch area along the crack was observed in the toy bin outside. The director removed and disposed of the cracked toy.
106(8): Safe Sleep classes were observed as incomplete or expired for 11 teachers. -106(3): Initial trainings have not been completed within 3 months by 3 teachers.
