0165(3)(a) Licensor observed 2 deep sandboxes the children were playing in during the visit that needs more sand. The sandboxes have a plastic piping that goes from one end to the other end of the box and one has multiple big rocks that are tripping hazards. 0165(3)(g) Licensor observed in the outdoor play area, are tables and play equipment with chipped paint and/or rusty parts. The play equipment has rusty, loose and hanging nails accessible to children. The slide has a small crack and there are a few tempered mirrors that have multiple little cracks.
Broken equipment observed stored assessible to children on top of the storage sheds, need to be repaired or removed or made inaccessible to children. Laundry room was not locked, open and accessible to children. Equipment on top of storage sheds, equipment in poor condition, need to be repaired, removed or made inaccessible to children. In toddler room, 1 outlet is not Tamper Resistant and is observed accessible to children posing a shock hazard to children.

Storage area under the kitchen sinks, had cleansers inside that were made accessible to children as the safety locks were broken and the kitchen door was left open with the kitchen unattended. Storage area was open and accessible to children. Kitchen was open and not locked, cabinets under the sink where cleansers and soaps are stored, had broken safety locks. Diaper wipes in preschool room saying keep out of reach of children, were accessible to children in a cabinet near the bathroom.
In the baby room, an outlet had a safety plug inserted, when removed by the licensor, black burn like marks noted to top of the outlet. Outlet needs to be replaced.
Diaper changing tables in 2 rooms, were observed not clean. Debris and particles noted under and around the changing pad.
Counter behind the kitchen sink, caulking was observed black, loose, and missing in places. Counter is no longer moisture resistant.
Assistant Director onsite during the visit, identified himself as being in charge in the Director's absent, Assistant Director does not meet the minimum hiring requirement as an Assistant Director. Assistant Director is missing Child Care Basic Training and required TB Test per WAC.
1 Staff has not completed basic training, 30 hours basic training. Out of 5 staff files viewed, 4 did not have FH cards to view to include the Director and supporting staff. In merit, several staff are either noted expired for FH card. Not all staff are current for BBP training to meet annual requirement by L & I. Assistant Director was working alone in a Toddler class and has not had Basic 30 Hours Training. 4 staff have expired safe sleep training as verified in Merit. 4 out of five files viewed, only one had CPR, FA, BBP as obsered on their phone. All files viewed, were missing documents and training certificates for staff.
Plastic bags were viewed accessible to children in the Preschool Room. Broken child's sink water faucet is no longer attached securely to the sink base and moves exposing a hole in which child can cut or have their fingers stuck into. In Toddler room, one electrical outlet plate is cracked/broken and poses a safety risk to children in the room. Large objects stored on top of outdoor wooden storage unit, pose a risk of falling on children who play below and around the storage unit. Various items viewed to include toys and brooms ontop of the unit. Outlet in Waddler Room, 1 is not TRR. Cell phone is Waddler Room observed plugged in an outlet next to the sink. Broken facet in Preschool room needs to be replaced, currently it does not work (no water flow) and is loose to pose a safety hazard to children.
Not all staff have records of Blood Borne Pathogens training completed. Not all staff have completed or have expired CPR First Aid Cards, staff records in Merit are not current to include one staff is a sub and has not worked for a year. That 3 staff have an expired FA CPR Card. Not all staff (3) have taken or have records available for Food Handler Training. Some staff have expired food handlers cards.
White noise machines observed attached to the cribs of 2 children. A child was observed with a binky (pacifier) attached to their sleep sacks which poses a choking hazard and is a safe sleep violation.
Butt paste diaper cream was observed stored accessible to children in the preschool room diaper changing cabinet. Butt paste diaper cream in the preschool room, did not have parent consent paperwork with it. No paperwork was observed at all for the diaper cream. .
Bottle prep counter, was viewed no longer sealed or moisture resistant as the counter has signs of water damage to the counter top.
Medication (epi pen & benedryl type medication) was present for a child with a known food allergy with no health care plan or paperwork in how to administer the medication, the name of the medications, directions and dosage amounts, and description of allergic reactions and symptoms associated with the child's particular allergies.
A child with an epi pen and benedryl type medication, had no written or signed consent in their file or with their medication. Salma Aweis, Evelyn Castro, and Heather Ives have not had medication management training.
Heavy objects and falling hazard in every room noted. Older Toddler room: cabinets not secured to the wall, piles of clutter stacked unsecured ontop of cabinets and counter tops, heavy items not secured to counter tops and window sills. Younger Toddler room: cabinet not secured to the walls, clutter unsecured ontop of cabinets, backpacks not secured observed on window sills. Baby Room: Tall cubby not secured to the wall. Preschool Room: Upper wall mounted cabinets full of heavy teaching supplies and equipment, latches are broken and pose a falling hazard risk to children below. Tall cabinets not secured to the wall observed. Heavy objects on countertops and cabinet tops not secured. Outside Play Area: Storage cabinets observed to have items stacked ontop of eachother all unsecured and poses a falling hazard risk. Center for use rooms had 2 fans present, both presenting a falling objects hazard. Washer and Dryer room door, no longer has a lock and is accessible to children, poses an entrapment hazard. Diaper wipe packages saying keep away from children due to choking hazard, were observed in all rooms accessible to children. Plastic bags in diapering station in the preschool room were accessible to children on a cabinet. Carpet in the younger toddler room, is frayed with a unraveled loose binding, poses a tripping hazard. One outlet in the older toddler room had one outlet that was not TRR. In the older toddler room and younger toddler room, devices were observed plugged in near the sinks.
Cleaning supplies and laundry supplies were observed stored in a Utility/Laundry room that has no locking system and is accesible to children. Lotions and 1 jar of vaseline were observed in the classrooms. Diaper creams were observed accessible to children in the older toddler room and in the preschool room. Storage room/Laundry room, was observed unlocked with locking system removed making the room now accessible to children.
