Staff reported during a fire alarm, all the classrooms had vacated the building, during head count they realized a child was left alone in the building.

Janitorial closet door was observed left open making items inside, accessible to children. Provider corrected during the visit by closing the door to secure the lock. Storage door was discovered to be left open and accessible to children. Provider corrected onsite by closing the door to secure the lock.

Not all staff have met annual BBP training. One staff as verified in Merit, had an expired food handlers card. One staff has an expired safe sleep training as verified in Merit.

1 medication in preschool one, did not have any written consent form present and with the parents signature. No written consent form was present with teacher guidance regarding the allergy and medication. All other medications in preschool one, had all required paperwork as observed.

All classrooms have been retrofitted to keep the blind cord from forming a loop. In the classrooms, all except one, the cords were observed out of the retrofit tube, therefore, forming a loop that poses a risk of strangulation. One room, one blind had no retrofit at all as it was missing. This cord also formed a loop that poses a risk of strangulation. Plastic bags and rubber gloves accessible to children observed in the bathroom, poses a risk of suffocation and a choking hazard. Diaper wipe bags saying keep out of reach of children, also accessible to children in the same bathroom.
