The teacher in charge of 7 children playing on the climber reported that she did not see the child fall, but heard a loud bong and then she went over to the child who had a knot forming on his forehead. The teacher in charge of the group reported that the child did not cry at the time or just recently after the fall. 4 other staff were cleaning up outside after graduation. It was reported that the by the staff in charge, that she was between the staff cleaning up tables and the climber where the children were playing. Mom reported from the window, she saw her son approaching the teachers being ignored to include the classroom teacher who was supervising the group of children.”

    • “valid as two staff both reported that they did not see the incident occur of children pulling down their pants during outdoor play. Staff 1 reported not to be in line of sight of the children, but over by the door to the toddler classroom. Staff 2 reported that he was putting away outdoor equipment into storage area and had his head turned away. When he turned around, he saw the 4 students, 3 of them were adjusting their clothing. All 4 students reported that 3 girls, pulled down their pants and one did not. It was reported that the younest children in care, was 5 years old.”

Diaper wipes that say keep out of reach of children, observed stored with the plastic bags, all accessible to children. Tall Children Wooden Cubby Cabinets have been moved to other walls in the classrooms and not reattached or secured to the walls, all pose a tipping hazard to children if climbed on. One room, the infant room, one handwashing sink tested at 130 degrees, other sinks in the room, tested 100-107 degrees. Multiple electrical cords observed plugged in the wall with an adapter that was not a surge protector in preschool classroom. One room, had a device plugged into an electrical outlet near a sink.”
