During the inspection, Licensor Harris determined that the staff member involved in this interaction did not practice guidance and interactions with the child in the Intake. At this time, staff member has been removed from the Center and is no longer a staff member. The following day, all staff members completed positive guidance and interactions training. Director reviewed policies with staff members and informed staff members to report concerning behaviors and instances to Leadership as soon as possible. (1)(c)
Based on the information gathered from the inspection, it was determined that the level of supervision should have been raised on 3/13/2024 and staff members should have planned accordingly to prevent similar incidents from happening again on 3/13/2024.

110-300-0215(3)(a)(i)- Medication had childs name labled on the ouside of a plastic ziplock bag and the childs name labeled on the actaul medicine bottles and epi pen with a label maker.

A child with asthma had expired medication.
The first-aid kit was observed to be missing the following item at the time of inspection: First-aid manual.
A large size fish tank without a physical barrier on top was observed in the common area (studio) where the water was accessible to children at the time of inspection.
One of the refrigerators in the kitchen area was measured at 46 degrees Fahrenheit at the time of inspection.
Large items were observed on top of floating shelves in the Vashon Island classroom that posed a risk of tipping at the of inspection.
A padlock was observed to be placed on the latch of a second emergency exit gate in the outdoor area (Blake Island) at the time of inspection.
