At the time of the inspection, this licensor observed an outdoor climbable slide that did not meet CPSC surfacing or safety fall zone guidelines. The woodchips underneath the slide measured to be four inches. Another non-climbable outdoor toy was measured to be within the slide's six-foot safety fall zone. Emergency preparedness plan

Staff files were missing sign off that they reviewed the Disaster Plan. Program did not have a 3 day supply of food and water. Need to ensure that the Disaster Plan has a picture of the floor plan with the water and electricity shut off.

Baseboard by entrance exit door was observed to be gone, chipping paint was observed on changing table, 1/2 doors into classrooms, and on wall in the preschool/school-age room. Also in the preschool/school-age room the licensor observed the white shelving units to hae some areas with chipping paint. The diaper changing table was also observed to have chipping paint.

Staff member, NFG brought children in from outside and put the chain across the door that leads to the outside. This is the entrance and exit for the program. In the Preschool/School-Age Room the door was being locked that exits the room.This was observed to be done by the children at the program.

Child was noted in health history to be allergic to bee stings. The child was noted to have an epipen but the epipen was not at the program. There was no other paperwork on file other than the noted information in the health history area of the registration regarding being allergic to bees. A child's toothpaste and sunscreen forms did not have end dates. The end date read n/a.

Door knob for child care was locked and requires an unlock of the knob and then a turn of the knob to get out. There is a deadbolt on the door as well but that was not locked.

Preservice requirements

MERIT needs to be updated to reflect staff that are no longer at site. AAA is listed with a PBC as "Withdrawn". PGD reads PBC status of "No Application Received" Provider stated she has been having trouble getting into MERIT attempted to get in but there was some maintenance happening in the system.
